JOB TITLE: Service Coordinator for Infant and Toddler Connection of Highlands

SCHEDULE: 40 hrs/per week – flexible schedule as needed
DUTIES: In accordance with Virginia’s Part C program for infants and toddlers with disabilities the service coordinator is responsible for guiding an IFSP team to coordinate services based on multi-disciplinary assessments and family priorities, obtain social and medical background information for infants and toddlers, develop, review and evaluate individual family service plans (IFSP), assist families in identifying and assessing needed services, maintain children’s records, coordinate needed services, initiate and complete activities necessary for child’s transition to pre-school services, and meet with other program staff to review individual family services plans, service coordination needs, and monitor program status.
POSITION REQUIREMENTS/QUALIFICATIONS: Bachelor’s degree in human services field with medical/pediatric experience helpful. Must be able to consistently and effectively provide concise, appropriate documentation. Have knowledge of community supports and services.

SCHEDULE: Three (3) 24-hour shifts (including on-call duties)
DUTIES: Provides pre-screening services; conducts initial assessments on persons needing immediate attention; coordinates treatment of services; develops individualized service plans; provides consultation on individuals’ functioning to community agencies. Applicants should have a working knowledge of the etiology and treatment of psychiatric disorders including patients with mental illness as well as substance abuse. Applicant will need to be able to work outside of normal business hours.
POSITION REQUIREMENTS/QUALIFICATIONS: Master’s degree in human services field required, Master’s degree in clinical psychology preferred. Also, previous certification as Virginia Certified Pre-screener beneficial, but will train.

SCHEDULE: 40 hours/week
DUTIES: Develops and monitors the Children’s Services program budgets, monitors and insures that all consumer records are compliant with licensure standards, Medicaid regulations and agency policies, promotes and maintains effective cooperative agreements and working relationships with local human service agencies, supervises program staff, recruits, develops and maintains a working relationship with the natural and foster parents, provides back-up for the 24/7 on-call services, completes various program reports, develops and implements program goals and objectives, assures mandatory training compliance for staff, implements and monitors the program’s Utilization Management Standards, assists the Department Director with various tasks, develops, implements and evaluates the program’s quality improvement plan, aids in the HCS strategic planning process, participates as a member of the Family Assessment Team (FAT) and Comprehensive Services Act/Family Assessment Team (CSA/FAT) activities, facilitates and monitors the consistence and appropriateness of services, provides technical assistance and/or back-up for emergency crisis intervention to consumers, and participates on HCS intra/inter-agency, local, regional and state committees and task forces to coordinate, plan, design and evaluate services for consumers served by the program.
POSITION REQUIREMENTS/QUALIFICATIONS: Master’s degree in human services or equivalent license and three (3) years related experience required.
