Developmental Services

DD Services

HCS Developmental Services provide a detailed person-centered plan to individuals with significant limitations in intellectual functioning and adaptive behavior before age 18, as well as infants and toddlers with delayed development from birth to age three. Our organization believes that within every disability is ability, and that a good life is possible for all individuals served in our programs.

Horizons Day Support Program

Horizons is both a center and community-based program for individuals with intellectual disability who are ages twelve years through adulthood. This program provides assistance to increase socialization, life skills, communication, and more through community resources (e.g., field trips and volunteer opportunities) and center-based activities (e.g., arts and crafts, personal hygiene education, and horticulture).

Infant & Toddler Connection

Early Intervention Services
Early Intervention supports families of infants and toddlers, ages birth to three years, with developmental delays and disabilities. Children learn best with people they know and in the places they spend most of their time.

Sponsor Home Program

The Sponsor Home Program offers individuals with a developmental disability the opportunity to live in a licensed sponsored family home in their community. Sponsor home settings offer a nurturing and supportive home environment focusing on supports to increase the individual’s independence, health, safety, and community involvement.

Support Coordination

Intellectual Disability Support Coordination provides case management services, as identified in a detailed Person Centered Plan, to assess the needs (medical, psychiatric, social, educational, residential, etc) of individuals with intellectual disability and coordinates, links, and monitors supports for those individuals.
